Actual pay rate dependent upon experience._ $34.00 - $51.00 **Position Highlights** **Position can be located in Sioux Falls or Rapid City, South Dakota **Student Loan Repayment:** This Registered Nurse (RN) position may be eligible for Avera’s Student Loan Repayment Program, which helps you pay off your loans and save money. Work where you matter.** **A Brief Overview** Responsible for coordinating clinical aspects of the transplant patient care. Maintains 24-hour accountability for the coordination of transplants or other patient coordination needs. Facilitates quality patient care for transplant patients, donors and their families. Participates as a member of an interdisciplinary transplant team. The Coordinator works with patients and their families beginning with evaluation and continuing through post-transplant phases, in a compassionate and professional manner in order to achieve excellence and continuity in-patient care throughout the phases of transplantation. **What you will do** + Coordinates all phases of the transplant with the referring primary and specialty care physicians and providers, transplant physician, consulting physicians, patient and patients’ family and hospital and clinic staff as needed for patient care coordination. + Assures all pertinent information in patient transplant status is communicated accordingly, and the appropriate intervention is implemented. + Follows the patient through all phases of transplant regardless of setting, to ensure continuity and coordination of care. + Use nursing judgement, attend outpatient visits and do inpatient nursing rounds as needed, to assure ongoing assessment and care planning. + Maintains appropriate monitoring and tracking documentation at the time of service to assure comprehensive and timely completion of tasks; sufficiently communicates to care team; sufficiently documents patient clinical status in designated databases to meet data collection requirements. + Coordinates patient referral intake and diagnostic screening to facilitate evaluation, selection and care of the transplant donor as appropriate. + Provides patient education related to, but not limited to, the transplant process and procedures related to that process. Responsible to make updates to patient education materials for currency and completeness. + Coordinates with support personnel to assure record retrieval, scheduling, and patient written correspondence as needed. + Provides clinical information as needed to the Patient Advocate for the payor approval process, including summary of patient data for the request for medical necessity approval letter. + Seeks recommendations from transplant psychosocial professionals in order to adequately develop and coordinate care plans. + Actively participate and collaborate, as assigned in the transplant committees to include clinical and PI committees. + Serves as a resource in the development of, and comply with, standard operating procedures and standards of care to meet regulatory and participation standards of JCAHO, FACT, CIBMTR, NMDP, and research protocols. **Essential Qualifications** The individual must be able to work the hours specified.
