Registered Nurse - Structural Heart Program Nurse - Day Location: York Hospital, York, PA Schedule: Full Time Schedule Full Time: 80 Hours/Biweekly Hours: Start Time 6:00am or 7:00am Monday - Friday; No Weekends or Holidays General Summary Organizes and performs a wide variety of administrative and fiscal functions for cardiovascular services across the system specific to the Transcatheter Aortic Valve Replacement (TAVR) Program. Partners with the medical, surgical and clinical staff of multiple departments, both inpatient and outpatient within the Hospital to coordinate the care of cardiovascular patients who are being evaluated for the TAVR procedure and post procedure care. Contributes to the overall operations of the TAVR Program, including inpatient oversight and outpatient management of TAVR patients, in conjunction with the TAVR Program physicians. Duties and Responsibilities Essential Functions: + Assists with coordinating the referral process and introduction of patients into the Valve Clinic for purposes of evaluation and management. + Guides and directs the continuum of care for valve patients from admission through discharge, including post discharge care and follow-up as appropriate. + Actively participates in data collection for TVT Registry to ensure that data related to valve patients is reported accurately and appropriately for purposes of Quality Improvement. + Actively participates in research projects that arise from the Valve Clinic as directed by the Medical Director and Administrator. + Coordinates and implements educational materials and support needed by the patient and family. Ensures that patients and families receive appropriate counseling to assist them to progress toward outcomes. + Provides daily follow up for hospitalized patients and documentation in the progress notes pre- and post procedure, as well as those undergoing screening tests and procedures. + Completes pre- and post procedural NIH Stroke Scale, KATZ scoring and KCCQ questionnaire as well as five meter walk test and grip score. Completes appropriate tests at one month and annual follow up. + Completes review of tests and images prior to the team meeting. + Conducts the pre- and post assessment TAVR clinic in collaboration with TAVR physicians from Cardiology and CT Surgery. + Assists with the screening process of all potential patients upon notification of referral to TAVR program. + Facilitates MD contact for clinical referrals, transfer requests and family/patient inquiries. + Designs necessary TAVR clinical power-point presentations for team meetings and proctor/sponsor approval. Finalizes with physicians and research department. + Assists Structural Heart Coordinator in developing monthly reports on referring physician volume, outcomes and TAVR volume. + In collaboration with nursing education, assists in developing a clinical competency study module. Qualifications Minimum Education: + Bachelors Degree Nursing (BSN) Required Work Experience: + 5 years Relevant experience. Required + 1 year Cardiovascular experience. Preferred Licenses: + Licensed Registered Nurse Upon Hire Required or + Registered Nurse Multi State License Upon Hire Required and + Advanced Cardiac Life Support Upon Hire Required Knowledge, Skills, and Abilities: + Excellent communication, interpersonal, organizational and computer skills.
