Job Description Summary:

Responsible for the Professional Case Management of the individual patient caseloads as assigned, by seeing that all care ordered by the physician is carried out and documented in a timely, efficient and professional manner. The RN performs acts which require substantial specialized knowledge, judgment, and nursing skill based on principles of biological, psychological, physical, and social sciences. He/She will service as liaison between patient, families, Call Center registered nurses, physicians and other ancillary service providers. Candidates must have the capabilities and skills to supervise and assist LVN’s. He/She will participate in regular staff meetings and educational programs as assigned and will complete yearly competency examinations as provided by the Clinical Director/ Agency Administrator.

JOB RESPONSIBILITIES:

Carries out job assignments on all patients assigned in a timely manner. Documents care given according to appropriate policies & procedures within specified time frames. Maintains accurate and complete records with pertinent assessment of patient condition and the home setting. Performs patient care according to agency policies & procedures. Communicates effectively with patients, family and physicians, which involves effective teaching as ordered. Provides appropriate reporting to other nurses.Complete assessment and Plans of Care (POC) accurately. Implement POC effectively and evaluate the effectiveness and outcomes. Revise POC as needed to reflect changes in the patient’s condition, responses to therapy and desired outcome(s). Utilize POC to improve individual patient care. Demonstrate via documentation: correlation between POC, Physician’s orders, and nursing notes.Coordinates patient care accordingly. Receives patient care assignment and contact patient to establish visit time. Coordinates medications and equipment ordered for the patient. Verify that all supplies, medication/pumps etc. are as ordered by physician.Consults with client and family regarding: Pathophysiology and disease process, self-care techniques and prevention, steps to take in emergency, adverse reactions to administered medications, ways to reach Healthy Connections, other resources about disease process, and seeking answers to questions from physician/other sources.Responsible and accountable for making patient care decisions that are reflective of the nurse’s experience as well as the patient’s educational preparation/level.Maintains confidentiality with all patient information and records.Receives and transcribes physician’s orders accurately. Verifies orders against medications and supplies received. Confirms all pump settings with another RN before initiating any pump therapies with patient.Admits patients using appropriate forms and paperwork on every patient admission. Completes consents for therapy as needed. Provide patient with insurance estimates (within a range). Offers patient consultation with insurance manager to discuss insurance issues. Follow-up/Re-teach visits with appropriate documentation and nursing notes.Participates in multidisciplinary team conferences and provides precepting, training, and mentoring to other nurses for orientation, and onboarding and supervisory activities as assigned.Demonstrate safety and competency with patient care activities.Maintain cost control in use of all supplies, equipment and medications.Completes company paperwork in a timely fashion.Documents all findings and reporting accordingly including complaints. Complaints, abnormal findings, and conditions to physicians, Nursing supervisors, and/or Administrator.

(i.e. hiring, recommending/approving promotions and pay increases, scheduling, performance reviews, discipline, etc.)

BASIC EDUCATION AND/OR EXPERIENCE REQUIREMENTS:

Graduate from an accredited School of Nursing.Current license by the State Board of Nurse Examiners.Two (2) years hospital-based Obstetrical/Women’s health experience.Certified in Basic Life Support (BLS) and Cardio-Pulmonary Resuscitation (CPR).

BASIC QUALIFICATIONS:

Ability to perform at a supervisory level.Current Driver’s license and proof of required insurance.Ability to communicate effectively with co-workers, medical staff, patients and families.

PHYSICAL DEMAND REQUIREMENTS

Ability to lift up to 50 pounds with a maximum lifting of 75 pounds. Exerting up to 20 pounds of force occasionally, or up to 10 pounds of force frequently. Physical demands may involve walking, standing, crouching, kneeling, turning, pivoting, balancing, stooping, reaching overhead, grasping, pushing, pulling, lifting and carrying. Fine motor skills and visual acuity required by this job include ability to see up close and from a distance, color and peripheral vision, depth perception and the ability to adjust focus.Team members in this job classification have the likelihood of occupational exposure to blood, body fluids and other potentially infectious materials. Possible exposure to hazardous substances with possible effect on reproduction, injury from needles, other sharps, fumes, chemicals, humidity, cold, heat, adverse weather elements, animals, secondhand smoke/vape and unpredictable home environments.Safety requirements include closed toe-shoes, facemask, goggles, gown and/or gloves and functioning device for communication in both routine and emergency situations.

TRAVEL REQUIREMENTS:

Willing to travel up to 100% of the time for business purposes.

PREFERRED QUALIFICATIONS:

One (1) year experience in Home Health provision, preferred.

Due to state pay transparency laws, the full range for the position is below:

Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data.

Pay Range is $38.08-$63.45
