Registered Practical Nurse Case Manager

What you do:

 

The Registered Practice Nurse Case Manager (Bilingual French) will report to the Medical Operations Excellence Director and oversee medical case management in a 24/7 setting, ensuring clients receive the care and support they need. Acting as a liaison between patients, families, physicians, and case management teams, and ensure effective communication between these groups. Manage international outpatient and inpatient cases, including medical reviews and records. Coordinate with providers and patients for medical information and support. Provide emotional support and exceptional customer service to families. Review medical history and information against case records including assessing primary care physician records for eligibility and pre-existing conditions. Assess stability and transport needs and develop medical direction and care plans. Participate in daily rounds and provide medical support. Consult on medically underwritten policies and questionnaires and collaborate with business partners on complex and escalated cases. This position is considered part-time where you will work 2 to 3 day, evening or night shifts a week.

 

What you bring:

 

College Degree/Diploma in Nursing with current registration as a Registered Practical Nurse. Minimum 4 years of nursing experience, preferably in acute care. Completion of a Physical Assessment course preferred. Strong attention to detail and multitasking abilities. Excellent verbal communication skills, including proficiency in French. Comfortable with extended phone-based communication throughout the shift. Commitment to ongoing professional development.
