+ JOB REQUIREMENTS + Education + High school graduate or equivalent. + Experience + Ideal incumbent will have direct patient care experience preferably with frail elderly/geriatric population, will maintain active certification as a Certified Nursing Assistant, or have completed approved vocational-technical program. May work under stressful circumstances at times. + JOB SUMMARY + The Rehab/Restorative Tech assists the rehab and activities team with treatment and care according to state, federal, professional, and PACE guidelines. In addition, the position: + Requires effective communication skills for working with participants, families, caregivers, and team members. + Requires competency in assisting with delegated treatment, activities, and care of participants. + Performs tasks as delegated by and supervised by licensed therapists and facilitates/leads group activities in conjunction with activity coordinator. + Supports an environment and culture that enables the PACE enter to fulfill its mission by helping to provide participant safety and participant-centered treatment or activities. + Maintains open and on-going communication with coworkers and supervisors. + Participates in team meetings an completes daily documentation/care plans per PACE guidelines.
