Job Description
**Summary**
Manages the care of a defined patient population across the continuum through collaboration with other care providers and
physicians. The case manager facilitates quality, cost-effective outcomes through appropriate intervention and utilization of
resources. Provides psychosocial assessment and intervention to address the emotional, environmental and financial barriers to
optimal outcomes and quality of life.
**Essential Duties and Responsibilities**
Performs psychosocial assessment within scope of license and monitors patient's emotional status, providing crisis intervention and
individual and family supportive counseling in the areas of coping and adjustment, making referrals as appropriate
Facilitates interdisciplinary treatment planning through patient care conference, ensuring input from the person served, monitors
progress and alteration in plan of care and assures appropriate services are provided.
Facilitates timely, appropriate communication between patient and all individuals in the continuum of care, and coordinates family
conferences. Functions as patient advocate.
Provides insurance reviews and pre-certification where appropriate, and serves as a liaison with insurance companies.
Performs discharge planning to ensure optimal, timely outcomes for individuals and their systems, facilitates transfers to other
facilities.
Coordinates care of individual served as it relates to reintegration into the workplace and/or community.
Participates in support groups in assigned areas.
Maintains awareness of financial issues and insurance benefits as well as cost of services.
Participates in the education of individuals served and their families during course of treatment and at follow-up if indicated.
Ensures appropriate utilization of resources given the clinical progress of the individuals served.
Demonstrates behaviors which foster a positive work environment.
Attends required meetings and in-services and adheres to hospital and departmental policies, procedures, Mercy Core Values and
ethical conduct.
Documents clearly and appropriately according to Hospital and program guidelines.
Maintains program productivity standards.
Performs other duties as assigned by Clinical Leader.
LSW, Bachelor's degree in Social Work and current Ohio license and one to two years related experience and/or training in a
medical setting.
MSW and knowledge of medical rehabilitation preferred.
