WORK SCHEDULE:

Must be available to work Monday to Friday between the hours of 8:00am to 5:00pm in the Eastern Time Zone.

Summary:

Remote LPN Triage Nurse is a licensed practical nurse with oncology-specific clinical knowledge that offers individualized care and clinical guidance to patients, families, and caregivers to assist with ongoing healthcare needs.

PRIMARY TASKS AND RESPONSIBILITIES:

Under general supervision, following established policies, procedures, and professional guidelines, provides care to patients by triaging oncology patient calls regarding treatment, surgery, and appointment information.Monitor and provide patient symptom management.Address barriers to care for patient, care giver, or family needs to achieve optimal patient outcome.Assist high risk patients with the goal of minimizing Emergency Department and inpatient readmissionProvide patient-centered individualized ongoing education, resources, and referrals to internal and external resources.Assist the physician and PA-C/ARNP with specific patient/family interaction needed to resolve clinical issues.Complete requested clinical documentation as neededEstablish and maintain professional role boundaries with patients, caregivers, and the multidisciplinary care team in collaboration with manager as defined by job descriptionFacilitate communication among members or the multidisciplinary cancer care team to prevent fragmented or delayed care that could adversely affect patient outcomes. Provides patient education by answering questions following chemotherapy, radiation, and infusion treatments and post-surgery.Additional tasks as needed to support team.Work as an integral team player and is expected to adhere to and abide by the rules and regulations set forth by the Florida State Board of Nursing.

EDUCATION/CERTIFICATIONS & LICENSES:

High school diploma /GED requiredActive Licensed Practical Nurse multistate or Florida single state licensure required

EXPERIENCE:

Three (3) years of LPN experience requiredTwo (2) years or more of oncology experience requiredOncology nurse triage experience required
