Summary:

Assists current and potential future residents of The Caring House and provides multi-faceted support. This will be done by acting as a key resource and providing information requested along with responding to questions/concerns from residents and/or their families. With a focus on CMS regulatory requirements and compliance adherence, assists residents in accessing services for which they are eligible including obtaining/utilizing community resources assistance.

Critical Tasks:

Acts as an advocate for TCH residents to ensure that their needs are met, while providing outstanding customer service.Provides information, explanations, and guidance on resources to residents and their family members.Coordinates with social workers to ensure that all information provided is accurate and consistent. Assists residents in applying for services they are eligible for and advocates on the residents’ behalf to other service organizations or agencies. Routinely meets with the residents at the Caring House to facilitate communication and identify the resident needs. Communicates and coordinates with transportation to arrange transportations as needed for residents during admissions and discharges. Assists with obtaining any necessary DME for residents who are discharging from services at The Caring House.Responds to any resident concerns or grievances and assists in getting them resolved timely.Assists The Caring House DLT with any CMS reportable events investigations.Makes visits to residents’ homes when necessary. Attends and actively participates in TCH resident council meetings. Assists and responds to resident concerns while communicating these concerns to staff and suggesting solutions. Performs other job-related activities as requested.Ensures that all critical tasks are performed on a timely basis.

Required Qualifications:

High School Diploma or GED required, prefer completion of some college course work.Prior experience in a customer service role ideally within a similar setting.Ability to calm irate, anxious, or grieving residents.Excellent interpersonal skills Strong verbal and written communication skills.Must be a member of a federally recognized tribe.Ability to communicate in Pima language preferred.The ability to obtain an AZ fingerprint clearance Card.Knowledge of CMS regulations or the ability to understand and interpret CMS regulations within six months of date of hire.Demonstrates ability to relate to diverse cultures and specifically the Gila River Indian Community and/or other Native American cultures and community health services.
