We are seeking a Licensed Clinical Social Worker (LCSW) to join our team at the Southern Utah Veterans Home. The ideal candidate will provide social work services to residents and their families, focusing on promoting their emotional and social well-being.

Responsibilities: Conduct assessments to determine residents' needs and develop individualized care plans Provide counseling and support to residents and their families Coordinate with interdisciplinary team members to ensure comprehensive care for residents Advocate for residents' rights and access to services Participate in discharge planning and transition services Qualifications: Master's degree in Social Work Licensed Clinical Social Worker (LCSW) in the state of Utah Experience working with older adults and/or veterans preferred Strong communication and interpersonal skills Ability to work effectively in a team environment
