Overview
The Residential Intake Coordinator plays a key role in supporting the residential leadership team and the Director of Advocacy Supports and Community Resources. This position is responsible for screening and coordinating internal residential placement transitions and assisting individuals who are not currently supported considering residential placement with Heritage Christian Services. The Residential Intake Coordinator will actively support and promote the mission, vision, and values of Heritage Christian Services.
Pay rate for this position: $27.50 / hr - $30.20 / hr
The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above range represents the organization's good faith and reasonable estimate of the range of possible compensation at the time of posting
Responsibilities
+ Assist in maintaining accurate information in the universal needs registry.
+ Serve as the screening committee chairperson and in that role fulfill the responsibilities and tasks outlined in standard processes for internal move, external move and discharge.
+ Provide clear and timely communication with screening committee members, residential leadership, person considering move, family/guardian, care coordination, OPWDD and MHLS as needed.
+ Complete vacancy announcements and submit to OPWDD within 5 calendar days of vacancy.
+ Complete and maintain all Site Profiles with OPWDD and submit updated profiles within 5 calendar days of any changes.
+ Responsible for management and oversight of internal move requests.
+ Screen all referrals within 14 calendar days and submit screening reports to OWPDD.
+ Complete assigned forms/notifications such as internal move form, request to fill, agreement letter, vacancy announcement.
+ Collaborate with resident accounts to ensure candidates income meets requirements if considering placement in a tax credit home.
+ Ensure accurate data entry into Choices database.
+ Schedule screening committee meetings, residential tours and visits.
+ Responsible to follow up with involved parties post visit, guide next steps and document outcome.
+ Schedule intake meeting with appropriate team members once placement is agreed upon.
+ Complete IT request for transfer or set up of account in service documentation and medical systems.
+ Validate all required documentation is completed and all applicable care plans are implemented prior to individual admission.
+ Schedule and facilitate 30-day post move-in meeting with Interdisciplinary Team.
+ Responsible for review and update of admission/discharge policies and standard work annually or as needed based on regulation and process changes.
+ Lead and facilitate self- advocacy group meetings
+ Attend ADRO and RM meetings as requested.
+ Attend monthly Vacancy Management meetings with DDRO.
+ Manage completion of Occupancy HCBS Setting Agreements.
+ Monitor CAS assessments completion and desired outcomes.
+ Complete compliance billing audits.
+ Serve in agency committees as assigned.
Qualifications
+ Residential experience required
+ At least three years’ experience in a position that serves people with intellectual and/or developmental disabilities required
+ Minimum one year of direct support experience is required.
+ Must demonstrate exemplary verbal and written communication skills
+ Ability to maintain composure and remain objective even when dealing with emotional topics, and facilitate crucial conversations when needed
+ Demonstrate organizational skills
+ Ability to create and maintain a positive and inclusive work environment built on respect
+ Experience using Microsoft Word, Outlook, and Excel, Therap, and ADP
+ Valid driver’s license that meets Heritage Christian motor vehicle standards
+ Physical ability to lift over 50 pounds and provide assistance to people who require assistance with transfers
+ Minimum of High School diploma or high school equivalency; college experience preferred
