Minimum Requirements:

At least 5 years of experience as a qualified professional, performing care management or case coordination to target population (children or adults with IDD/MH diagnoses).

Strong knowledge base of IDD services, including housing options, state/federal regulations, and community resources.

Excellent organizational and time management skills.

Strong interpersonal and communication skills to effectively build relationships with individuals referred, families, guardians, MCO and external providers.

Familiarity with person centered planning and individualized service plans.

Proficiency with Microsoft programs.

A Qualified Professional (10A NCAC 27G .0104) must have one of the following:

Bachelor degree (non Human Services) with 4 years Full time OR 8 years Part time applicable experience required

Bachelor degree (Human Services) with 2 years Full time OR 4 years Part time applicable experience required

Master degree (Human Services) with 1 year Full time OR 2 years Part time applicable experience required

This Opportunity:The Residential Placement Coordinator, LTSS is responsible for coordinating screening, touring, and preparing individuals for waitlist across Monarch residential programs. This role involves assessing needs, identifying suitable housing options, collaborating with residential teams, and ensuring compliance with state and federal regulations. The coordinator serves as a liaison between families, caregivers, community resources, and residential programs to facilitate a smooth transition and ongoing support for clients.

What You'll Do:

Assessment and Coordination

Conduct comprehensive assessments of individuals, including residential needs, preferences, and goals in collaboration with the person, family, and support team.

Match individuals to appropriate locations based on assessments.

Develop transition plans to waitlist and from waitlist to admission when appropriate.

Collaboration and Communication

Act as primary contact for assigned individuals, families, guardians, and support teams regarding residential waitlist and placement.

Work closely with residential leadership, care managers and community resources to coordinate meetings/tours and placement options.

Facilitate and lead communication to prepare for placement on waitlist for various locations.

Compliance and Documentation

Maintain detailed records of waitlist, assessments, and communications.

Knowledge of regulations and standards for each residential program.  Remain up to date on changes in services delivered and funding available to each program.

Ensure placements are appropriately matched in compliance with regulations, safety, accessibility, and quality of care.   

Advocacy and Support

Advocate in the best interest of individual referrals, program participants as it relates to appropriate placement and services.

Address any concerns or questions from families or team members and provide resolution to barriers encountered. 

Identify gaps that get in the way of waitlist development for each program and work to develop strategies that fill these gaps.

Assists in the implementation of the agencies strategic plan.

Meets regularly with management staff to review, discuss, revise, and enhance policies, procedures, and program design as necessary.

Coordinate with Marketing and Outreach Resources for external visibility and awareness.

Demonstrate knowledge of emergency procedures and assist in crisis situations.

Demonstrate knowledge of and comply with all agency policies and procedures.

Complete all other relevant responsibilities as assigned by the supervisor.

Driving and travel will be required.

Education We're Looking For:Bachelors: Human Services, Bachelors (Required), Masters: Human Services

Certifications We're Looking For:Drivers License (Valid) - USA, Qualified Professional (QP) - Monarch-DSM

Experience We're Looking For:Experience as qualified professional, performing care management or case coordination to target population (children or adults with IDD/MH diagnoses)

Schedule:Monday-Friday (8:00am-5:00pm)

Target Weekly Hours:40
