Monday-Friday 8am-4:30pm Requirements: + Bachelor degree in Human Services field (ore related field) w/ (1) yr experience working w/ DD population + Knowledge of community supports available to persons w/ developmental disabilities + Ability to interpret OPWDD regulations & Mental Hygiene Laws as they apply to operation of pro. + Valid NYS Driver’s License & approved record Rate of Pay: $21.00-24.00 per hour As a Residential QIDP, you’ll also receive: + Great work-life balance + On the job, paid training + Automatic & generous PTO + 10 paid holidays + No nights, no weekends and no holidays + Opportunities for advancement + On-demand pay Job Description Job Title: Residential QIDP Department: Residential Job Grade: 4 Job Code: N/A Position Summary: Under supervision of the Director of Residential Services, the Residential QIDP will be responsible for the quality and billing services including but not limited to: compliance with OPWDD and OMIG regulations, monitoring and certifying of individualized program planning, Staff Action Plan and Life Plan training, and development and revision of procedures as necessary. Reporting Relationship: Under general supervision of the Director of Residential Services, provides monitoring and oversight of individualized program planning. Job Duties and Responsibilities: + Service Management and Coordination + Assures that State and Agency regulations regarding Life Plans and Staff Action Plans are being met. + Monitors and tracks individual Life Plans, Staff Action Plans and Level of Care Eligibility (LCED) to ensure accuracy and completion. + Monitors individual staff action plans to ensure such plans are consistent with the person’s needs, preferences, and personal goals to make them to live, work, socialize, and participate in the community. + Track and Certify individual staff action plans to ensure they are completed in a timely fashion. + Assures that all plans address individual participants needs/safeguards consistent with the Life Plan. + Assist with training management on staff action plans, monitor implementation and meet with management as needed to evaluate individual’s progress. + Responsible for regular communication and visits to sites to ensure implementation of plans are being met. + + Provide regular review of data and individual progress. Assist in revising program goals based on individual's needs and performance, as necessary. + Reviews monthly progress notes for participants to ensure that they reflect the person’s response to services provided and to assure compliance with OPWDD funding requirements; Tracks to ensure all progress notes are done contemporaneously to service provision. + + Ensure timely completion of all annual assessments. + All other duties as assigned. Qualifications and Education Requirements: Bachelor’s Degree in one of the following professional categories (including, but not limited to) human services, sociology, special education, rehabilitation, counseling, psychology or related field required. O ne-year experience working directly with people with intellectual disabilities or individuals with physical and/or related developmental disabilities and co-morbid psychiatric diagnoses. PLUS Knowledge of community supports available to persons with developmental disabilities with an emphasis on residential supports/programs. Ability to interpret and implement OPWDD regulations and Mental Hygiene Laws as they apply to the operation of programs. A valid NYS Driver’s license and approved driving record is required. Vehicle in good operating condition. Maintain a professional attitude and appearance to at all times. + Equipment Operation: The ability to operate business equipment; telephones, fax machines, copiers, calculators, etc. Proficient in the operation of the computer’s hardware and software including applications and programming.
