90% FTE, 36 hours/week. 8 or 10-hour day shifts scheduled between the hours of 6:15 AM – 1:45 PM. Hours may vary based on the operational needs of department.

We are seeking a Respiratory Therapist Discharge Planner to:
Collaborate with the Nurse Case Manager and the entire care team in the development of care standards and guidelines for improving quality of care, collaborating discharge, and reducing re-admissions. Offer clinical expertise in assessment, recommendations and implementation of care plans/equipment.

Qualifications Graduate of a Commission on Accreditation for Respiratory Care (CoARC) accredited School for Respiratory Therapy Required Work Experience 3 years experience as a staff therapist Required Clinic experience in the adult or pediatric population Required Experience with discharge planning and coordination of respiratory durable medical equipment for the home Required Licenses & Certifications Licensed as a Respiratory Care Practitioner (RCP) in the State of Wisconsin Upon Hire Required CPR/BLS Certified Upon Hire Required Registered Respiratory Therapist (RRT) Credentialed by the National Board for Respiratory Care (NBRC) Upon Hire Required Our Commitment to Diversity and Inclusion UW Health is committed to being a diverse, equitable, inclusive and anti-racist workplace and is an Equal Employment Opportunity, Affirmative Action employer.
