Reviews, evaluates, and consults with patients and other health care providers on issues relating to drug related problems and optimal drug therapy.
Checks to prevent allergic or adverse drug reactions and reports them through appropriate channels.
Reviews and interprets medication histories in order to make recommendations regarding drug therapy, or the process of drug delivery to other health care providers.
Reviews and evaluates drug related information to make recommendations regarding the most effective, cost-effective drug therapy. Reviews and accurately interprets physician orders
Clarifies the patient's understanding of their therapy to ensure the best possible outcome. Assures medication needs of patients are met throughout the continuum of care.
Prepares and distributes drugs in accordance with Wisconsin Administrative Code relating to the practice of Pharmacy, providing appropriate adjunctive information to the patient or intermediary.
Provides work direction and guidance for technicians in performing technical pharmacy functions. Is responsible for checking technician's work to maintain high standards of pharmaceutical care.
Provides input into establishing and implementing written policies and procedures for all pharmacy operations.
Assumes responsibility for operation of the department in the absence of the direct supervisor.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
Pharmacist license issued by the state in which the team member practices, and
Patient facing pharmacists are required to be immunization certified. Immunization certification is to be granted from the state of WI or if from out of state, the program is approved by the accreditation council for pharmacy education (ACPE) or by the WI pharmacy examining board.
Education Required:
Bachelor's Degree in Pharmacy.

Strong customer service and organizational skills and confidentiality required.
Excellent communication (written and verbal) and interpersonal skills.
Demonstrated ability to work effectively with providers, staff and patients.
Must be detail-oriented with the ability to handle multiple demands simultaneously.
Intermediate computer skills including the ability to navigate through a database and run reports.
Life support training courses may be required dependent on department discretion.
Physical Requirements and Working Conditions:
Generally exposed to normal office environment.
May occasionally lift up to 25 lbs. without assistance.
Must be able to stand up to 100% of workday.
Must have functional speech, vision, and hearing.
Will be exposed to chemical hazards.
Operates all equipment necessary to perform the job.
