RN, Staff Nurse
Position Summary The registered nurse is responsible for assessment, planning, intervention and evaluation of the total care for clients receiving Adult Day Care Services. On assigned shift, the registered nurse is responsible for the coordination of interdisciplinary care, and works collaboratively with members of the care team. Minimum Qualifications Work Experience Two years of nursing experience in an acute care hospital or home care agency required. Previous Adult Day Care experience preferred.
Education Graduate of a Nursing program required. Bachelor's degree preferred.
Certifications/Licensure Kentucky RN license (OR) RN license in a State recognized by the Kentucky Nurse Licensure Compact Act required effective June 1, 2007. Job Specific Performance Standards The duties listed below are a summary of the major essential functions of this position.   Assesses needs, plans, coordinates, implements and evaluates nursing care of clients. Completes admission and/or transfer process, including client/caregiver/guardian teaching and appropriate documents. Assesses all new clients within 3 hours of admission. Maintains accurate and complete client records, utilizing all appropriate forms. Documents pertinent client information to include overall physical and emotional status, complete vital signs, self-administration of medications, self-injections and diet acceptance. Initiates plan of care on all clients within three (3) working days of admission and maintains current plan. Reviews and assesses plan of care every 90 days, or more frequently when client’s condition or goals change Initiates and maintains monthly summaries which are complete and timely. Administers direct client care including nursing care and client hygiene; and assists with van transportation, meal preparation, activities, and other client needs as directed. Administers medications ordered by the physician accurately and on time, and supervises client self-administered medications as appropriate. Understands and adheres to correct techniques as described in the ADC policies and procedures or professional references, when performing or supervising procedures such as self-medications, colostomy care, or proper injection techniques. Provides education to clients, caregivers and/or family members, and meets needs and requests promptly and courteously. Establishes positive relationships and works collaboratively with community agencies such as home care staff, physicians and their office staff, therapists, case managers, and transportation providers. Follows standard precautions, and utilizes personal protective equipment, engineering and work practice controls to eliminate or minimize employee exposures. Receives, enters, sends off, signs off, physicians’ orders in a timely manner and ensures appropriate follow through including physician signature. Demonstrates knowledge and proper use of equipment necessary to perform job function. Maintains a neat, clean environment in the center. Assists in orientation of new personnel and others, such as students and volunteers.
