RN - ACT Nurse Care Manager

Spurwink is looking for a collaborative, engaging, and driven Registered Nurse (RN) with unencumbered Maine licensure to join our vibrant, interdisciplinary team serving adults with serious mental illness and substance use disorders in the Bridgton area.\n

The ACT Team Nurse works across programs to address primary prevention, health promotion, and population health across Spurwink's adult behavioral health programs by:\n\nProviding nursing consultation to staff, interpreting health data, and helping to integrate health and wellness goals into client treatment plans.\nNursing care includes a focus on community outreach, community integration, education, and time-limited patient-centered care aiming towards promoting independence in managing and maintaining optimal health.\nImproving communication and collaboration with all care providers across the healthcare spectrum.\n\n

This is a full-time salaried position with no nights, weekends, holidays, or on-call work required. The pay is $32.00-$38.00 per hour depending on experience.\n

QUALIFICATIONS:\n

Minimum Educational Requirements: Maine licensure as a Registered Nurse (RN).\n

Minimum Experience Requirements: Three years of post-licensure experience in community health nursing, family nursing, pediatric nursing, hospital, or psychiatric nursing preferred. Direct service experience working with individuals with severe and persistent mental illness, serious emotional disturbance, and developmental disabilities is preferred. Must have the ability to exercise professional judgment in making decisions, communicate effectively with service recipients, family members, case managers, administrators, community providers, and other selected personnel. Must possess a high level of organizational and time management skills. Must be computer literate and able to navigate an electronic medical health record, utilize HealthInfoNet, generate PowerPoint presentations, and present basic data summaries to management. Knowledge of psychosocial, mental health, and vocational assessments, counseling theory such as MI, CBT, and DBT, physical and psychological effects of mental illness and substance abuse preferred.\n
