The Critical Care, Anesthesia, Perioperative, Extension (CAPE) and Home Ventilation Program is a multidisciplinary team of clinicians, respiratory therapists, and social workers committed to improving the quality of life for children who require long-term ventilator assistance at home. The program creates personalized care plans for children with complex medical needs and coordinates care with primary care providers, surgical specialists, home care nurses, medical equipment companies, and community services.
The RN will be responsible for:
1. Patient Care Coordination
+ Ensuring the comprehensive execution of established patient care plans, overseeing all aspects to ensure that each patient receives the care they need.
+ Assisting patients and their families with care directives, including attending consultations, arranging appointments, and coordinating lab work or imaging.
+ Navigating both internal and external healthcare systems to manage patient referrals and care coordination.
2. Medication, DME, and Authorization Support
+ Assisting with prescription refills and overseeing the prior authorization process for medications and medical equipment, ensuring timely access to necessary treatments.
+ Coordinating with Durable Medical Equipment (DME) companies to ensure the timely delivery of essential medical equipment.
3. Order Management
+ Facilitating the processing of orders for lab tests, medical referrals, equipment, and required documentation, including school letters, health forms, FMLA paperwork, and grant application letters of support.
4. Test Results and Patient Education
+ Reviewing and interpreting test results, offering relevant medical advice and education to patients and their families under the guidance of physicians and nurse practitioners.
+ Responding to patient inquiries, collecting additional details as necessary to provide accurate and effective guidance.
5. Communication and Collaboration
+ Serving as a liaison between patients, their primary care providers (PCPs), and other healthcare professionals to ensure coordinated, effective care.
+ Fostering strong communication between all involved parties to optimize the patient’s care experience.
6. Program Support
+ Assisting with the intake of new patients, ensuring appropriate referrals to specialists or necessary programs.
+ Supporting the transition of pediatric patients to adult care programs, ensuring smooth and efficient handoffs.
7. Insurance and Financial Coordination
+ Navigating insurance-related issues, advocating for patients to secure necessary coverage, and resolving any care barriers related to financial or insurance matters.
To qualify, you must have : :
+ Valid Registered Nurse (RN) license in Massachusetts.
+ Minimum of 5 years of nursing experience working with medically complex patients, preferably in a clinical or care coordination role.
+ Strong organizational and communication skills, with the ability to multitask and manage competing priorities in a fast-paced environment.
+ Experience with electronic health records (EHR) and other healthcare-related software.
+ Familiarity with insurance processes, prior authorizations, and care plan management.
Preferred Skills:
+ Experience in patient education and providing medical advice.
+ Familiarity with care transition processes, particularly transitioning pediatric patients to adult care programs.
+ Proficiency in collaborating with multiple stakeholders, including patients, families, healthcare providers, and insurance companies.
