The Diabetes and Endocrinology Clinic uses a multidisciplinary approach in treating endocrine diseases and disorders. We also offer a special program for those with diabetes. Our team of subspecialists provide patients access to new, innovative thinking and opportunities to participate in clinical trials.

The Diabetes and Endocrinology Clinic evaluates, diagnoses and treats a broad spectrum of endocrinologic and diabetic issues. Our patients receive complete, coordinated care through our highly trained care team.

What you will do as an RN Care Coordinator:

Coordinates care across the health care continuum within designated sub-specialty population(s)Serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and servicesCompletes pre-visit planning and post-visit wrap-up and determines vital follow-up appointments and/or treatments to optimize patient visitsFacilitates proactive disease management and preventative health care using evidence-based practiceRooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed.

What you will need:

Current Minnesota RN licenseBLS certificationDemonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacy
