Responsibilities: Provide comprehensive and individualized access to physical health, behavioral health, and supportive community and social services, ensuring patients receive the right care in the right setting at the right time. Coordinate the plan of care. Utilize population-based tools to support and monitor wellness and care goals for each patient, aimed at preventing illness and improving individual well-being, clinical outcomes and quality of
life while managing multiple chronic illness conditions. Coordinate care support self-management of patients with complex, chronic medical and psychosocial issues.
Utilizes current nursing practices to provide patient care while focusing on the needs of the patient and exemplifying the service excellence goals of care, concern, respect and cooperation. As a contributing member of a health care team, develops and provides a comprehensive plan of care for assigned patients.

Minimum Experience Required: 6-10 years of previous job-related experience with 3-5 years in sub-acute, acute
inpatient rehabilitation or similar setting.
Experience Details: Ambulatory or Case Management Experience: 1-2 years Licensure/Certifications: Required:
Current Registered Nurse License State of Illinois. Desired: national certification in case management.
Pay Range: $68,432 - $118,684.80 per year
Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
