As an RN Care Coordinator, you will provide a deliberate patient and family-centered partnership with the clinic team by developing and supporting a coordinated plan of care between visits (inpatient and outpatient) to promote a smooth transition across the healthcare continuum.

Our Vascular Clinic consists of a multidisciplinary team of specialists, including vascular surgeons, interventional radiologists, cardiologists, and nurse practitioners, who work together to provide personalized and comprehensive care. Our vascular clinic is striving to improve vascular health, enhance patient quality of life, and prevent serious complications through early detection, effective treatment, and ongoing management!

  

What you will do as an RN Care Coordinator:  

Coordinates care across the health care continuum within designated sub-specialty population(s)Serves as a single point of contact by facilitating and overseeing transitions in care within and between care settings, providers, and servicesCompletes pre-visit planning and post-visit wrap-up and determines necessary follow-up appointments and/or treatments to optimize patient visitsFacilitates proactive disease management and preventative health care using evidence-based practiceRooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed. Performs other duties as determined by manager and clinic needs

What you will need:

Current Minnesota RN licenseBLS certification3+ years of clinical nursing experience.  Demonstrated experience in patient advocacy and education, coaching, and proactive disease managementEMR and keyboarding/computer skills

Location:  909 Fulton St SE., Minneapolis MN

Hours: .8 FTE, 32 hours per week

Compensation:

75,525.00 - 117,063.00 USD Annual
