RN Care Manager

Regions Hospital is seeking an experienced RN to join our Care Management department in an RN Care Manager position picking up 2 shifts per month. 

This position provides inpatient vs Hospital care management services for medical/surgical patients to facilitate optimal patient outcomes, supports the delivery of effective and efficient care, improve continuity of care and efficient use of resources to progress the plan of care.  Focuses on developing a discharge plan to prevent readmissions and patient focused. The RN Case Manager will collaborate with the care team including, but not limited to: attending/specialty physician(s), Social Work, Utilization Review, therapies, and Nursing to ensure that an effective and timely treatment plan is acted upon, and appropriate discharge plan is implemented with a smooth transition in levels of care.  Provides Care Management services on weekends and/or holiday(s) rotations as determined by leadership.

Work Schedule:

Per Diem required 2 shifts per month

Required Qualifications:

Associate’s degree of nursing with RN licensure from an accredited college  One (1) year working as a registered nurse.   Current State of Minnesota Nursing Board RN license

Preferred Qualifications:

  Bachelors or Master’s degree of Nursing  Three (3) years’ experience. Experience performing, case management in Home Care, hospital or Post-Acute Care, discharge planning, care and/or quality improvement.  Certified Case Manager through Commission for Case Manager Certification (CCMC) or eligibility to sit for CCM exam within 2 years of hire.
