Per-Diem Care/Case Manager RN at Houghton Hospital

Coordinates the overall interdisciplinary plan of care for a patient, from admission to discharge. Acts as a liaison between patient/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. A certain degree of creativity and latitude is required. Typically reports to a head of a department/unit. Associates degree required, bachelors degree prefer. Must be a registered nurse with at least 2 years of clinical experience. Familiar with standard concepts, practices, and procedures within a particular field.
Position Qualifications/Minimum Requirements
Education:
Minimum - Associate's degree; Master's/Bachelor's degree preferred in nursing.
Experience:
Required: 2 years of clinical/hospital experience or 2 years of experience in related area.
Preferred: Five (5) years’ experience in Case Management
Skills:
Ability to read, communicate, and document effectively through oral, written, or electronic means. Strong organizational and customer service skills. Ability to interview others and assess situations, prioritize and respond promptly to a changing workload.
Licenses/Certifications:
Required for RN: Current Arizona RN license or compact state license
BLS Basic Cardiac (American Heart Association or American Red Cross)
