*Position Summary *
The Registered Nurse Care Manager collaborates with providers to identify, screen, track, monitor, and coordinate the care of patients with multiple chronic conditions and develop and maintain a system based comprehensive care-plan. Our Registered Nurse Care Managers seek to develop, implement, and deliver extensive care coordination within our medical group and affiliated partners in an efficient manner. This position interacts and collaborates with interdisciplinary care teams, population health teams, networked facility partners, physicians, and others involved in meeting the patient’s needs. This position enhances the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through coordinated care management.
*Primary Accountabilities *
1. Build a trusting relationship with primary care providers, internal care center team members, and our patient population
2. Responsible for managing multiple patient caseloads.
3. Conduct remote patient assessments in a designated time frame to include health status, care gaps and barriers to health success
4. Collaborate effectively with integrated care team to establish an individualized plan of care for patients
5. Engage patient, family, and caregivers to assure a well-coordinated treatment plan is established and executed
6. Provide education to customers on health conditions to assist with self- management goals
7. Make appropriate referrals to resources for collaborative care initiatives
8. Perform comprehensive assessments and documentation in a manner that is compliant with regulations. Understand and utilize the Athena and Privia Applications to determine high value opportunities and enter timely, accurate, and appropriate data.
9. Provides ongoing support and expertise through comprehensive assessment, care coordination, care plan implementation and overall evaluation of individual patient needs while ensuring patient preferences. Works as an extension of the primary care provider who is a member of the Accountable Care Organization (ACO) and is accountable for the health outcomes and health care expenses of a defined patient population.
*MINIMUM QUALIFICATIONS*
* Education: Associate Degree in Nursing or equivalent.
* Licensure: RN with the state of FL.
* Certification: None required
* Work Experience in Related Field: 2 years clinical experience.
* Work Experience in lieu of education: 4 years clinical experience.
* Knowledge/Skills/Abilities:Demonstrated ability to remain highly organized and detail oriented.
o Ability to solve complex problems; critical thinking skills; emotional and social intelligence.
o Strong verbal communication skills.
o Strong motivational interviewing and assessment skills.
o Possess planning, organizing, conflict resolution, negotiating and interpersonal skills.
o Maintain a positive caring attitude towards patients and team members.
o Independent problem identification/resolution and decision-making skills.
o Must be able to prioritize, plan, and handle multiple tasks/demands simultaneously.
o Ability to function independently and efficiently in a virtual environment and meet defined goals.
o Demonstrate computer application skills using tools such as EMR, MS Excel, Word, Outlook, and PowerPoint.
o Must have the ability to maintain confidential organizational knowledge. **
*PREFERRED QUALIFICATIONS*
* Education: BS in Nursing.
* Work Experience in Related Field: 5 years acute care experience.
* Knowledge/Skills/Abilities: Experience working in a virtual clinical environment.
*PHYSICAL REQUIREMENTS *
* Majority of time involves sitting or standing; occasional walking, bending, stooping.
* Long periods of computer time or at workstation.
* Light work that may include lifting or moving objects up to 20 pounds with or without assistance.
* May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise.
* Communicating with others to exchange information.
* Visual acuity and hand-eye coordination to perform tasks.
* Workspace may vary from open to confined, on site or remote.
* May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle.
