Founded in 1906, McLeod Health is a locally owned and managed, not for profit organization supported by the strength of more than 900 members on its medical staff and more than 2,900 licensed nurses. McLeod Health is also composed of approximately 15,000 team members and more than 90 physician practices throughout its 18-county service area. With seven hospitals, McLeod Health operates three Health and Fitness Centers, a Sports Medicine and Outpatient Rehabilitation Center, Hospice and Home Health Services. The system currently has 988 licensed beds, including Hospice and Behavioral Health. The Care Manager facilitates physician led, data driven, evidence based, outcomes-oriented quality improvement through a multidisciplinary committee. This includes the development of order sets. Job responsibilities include those listed in competency document. Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values. Utilizes the problem-solving process to improve care provided to patients as identified in the annual clinical effectiveness plan. Expert in problem solving methodology, reliability theory, and change theory, and utilizes all in generating solutions to care opportunities. Works collaboratively with the medical staff, departmental leadership, accreditation and regulatory agencies, quality clinical informaticist, and multidisciplinary teams to identify quality improvement opportunities, prioritize needs, and facilitate implementation of effective evidence-based care and design workflow. Demonstrates an understanding of A3 thinking. Other duties as assigned.
Qualifications/Training:
Five years of clinical experience required.
Experience with Microsoft Word, Excel, PowerPoint, and electronic medical record
Education/Certifications/Licenses:
Bachelor of Science in Nursing (BSN) required.
Master’s degree in nursing, business, or other health related field, or enrolled in a master’s program within 18 months and completed within 4 years of accepting position.
Licensed as a registered nurse in SC or other compact state
