Responsibilities:
The responsibility and accountability to handle all referrals within the skilled nursing facilitiesManages those coming into the building in the sub-acute unitManage referrals via multiple communication platforms (fax, phone and online)Collaborate and communicate with nursing professionals as well as insurance companies to manage insurance verifications and prior authorizationsContinual review and authorization of discharge planning, ordering supplies and setting up referrals, etcManage a strong relationship and partnership with Nursing LeadershipAssist on the unit ass neededWhat you will need:
Required:
A current license to practice as a Registered Nurse (RN) in the State of New YorkAssociate’s degree in nursing or Degree of Nursing from an accredited school3 years or more working as a RN ideally in long term carePreferred:
1 year of experience or more in a long term care facility or home care as a RNPrior experience in case managementBachelor's degree of nursingHighest level of customer service while completing day to day duties and responsibilitiesExcellent communication in a fast pace work environment across teams on different shifts (RNs different facilities and department) dieticians, insurance companiesWork independently as well as have strong engagement within a team setting to collaborate with others in the facility as well as externallyManage multiple tasks and can prioritize the most important tasksComputer literate and can learn multiple different computer applications and how they relate to each otherCritical thinking and can think on their feetPassion to learn, grow and develop in the nursing field
Pay Range: $34.20 - $48.45 per hour
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
