RN Cariopulmonary Rehab Supervisor Boulder Community Health, Boulder, CO 80303 $37.89 - $56.83 an hour - Full-Time Days, 40 hours per week Boulder Community Health is seeking a RN Cariopulmonary Rehab Supervisor who is responsible for development and implementation of all policies and procedures relative to Cardiac and Pulmonary Rehabilitation. This includes evaluation, supervision, and treatment of all patients enrolled in Cardiac and Pulmonary Rehabilitation. Participates in the establishment of program goals and objectives, quality assessment, problem solving, and marketing. Able to work with patient ages adult to geriatric. May be a point of care trainer. •Assumes responsibility for each patient during the course of treatment. •Develops an integrated treatment plan by assisting patients in goal setting and by gathering objective data that promotes basis for professional plan. •Assures that the person served is involved on an ongoing basis in discussion of plans, goals, status, etc. •Assures that the patient/family is adequately oriented to the program, that the program proceeds in an orderly, purposeful, and timely manner, and that discharge decisions and arrangements for follow-up are handled properly. •Monitor patients before, during, and after exercise according to AACVPR/Medicare patient exercise guidelines. •Refers patients to other services or outside agencies when patient goals cannot be met within program framework. •Evaluates patients as to the quality of care and compliance to program through patient staffing, observations, and surveys. •Develops curriculum, teaches classes, and assesses patient learning. •Establishes patient goals and objectives. Documents treatment plan & progress in EMR •Performs the technical/clerical tasks required of the job legally and safely related to Boulder Community Hospital policies and procedures.
