Jet Health, Inc / Klarus Home Health is seeking compassionate and dedicated Registered Nurses (RN) Case Manager to join our team. We are looking for full time, part time, PRN / Per Diem RNs who are passionate about providing quality home health care to our community-based patients in Aurora, Centennial, Glendale, Englewood and the surrounding areas.

Requirements Current State or Compact Registered Nurse (RN) License in handA minimum of one-year clinical experience is preferredOASIS knowledge highly preferredDemonstrate knowledge and skillset in current nursing practicePossess a valid state driver's license and automobile liability insuranceHCHB EMR knowledge is desiredFollow a designated care plan in accordance with patient's needsMake home visits to clients in designated geographic territoriesPerform assigned duties, including administration of medication, wound care, treatments, and proceduresMonitor clients' conditions, reporting changes to Clinical or Client Services ManagerFollow up with, execute, and properly document doctors' ordersPerform client assessmentsCase management and coordinationAccurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet
