Job Description

We're seeking a licensed RN to join our healthcare team as a Service Coordinator in El Paso, TX. In this role, you'll manage care plans for members with chronic illnesses and disabilities, conducting assessments and coordinating healthcare services to ensure optimal outcomes.

Key Responsibilities:

- Conduct holistic member evaluations (telephonic and face-to-face)

- Develop and manage individualized care plans

- Coordinate with healthcare providers, families, and community resources

- Monitor service delivery and ensure member satisfaction

- Advocate for patients and families within the healthcare system

Required Qualifications:

- Current unrestricted RN license in Texas

- Bachelor's Degree

- CCM/RUG Certification

- 1+ year experience in service coordination/case management

- Bilingual (Spanish/English)

- Valid driver's license and reliable transportation

- Must reside in El Paso, TX area
