RN Case Manager
Amergis Staffing is currently seeking RN Case Manager for TRAVEL or LOCAL assignment in Columbia, MD! Position Details: + Facilities Based Case Manager: Job requires you to float in Howard County & Southern Carroll County, MD to long term care facilities, hospitals, etc. but there is a weekly meeting every Wednesday at “home base” at Gilchrist Inpatient Hospice in Howard County - 6336 Cedar Ln, Columbia, MD 21044 + Client stated they would be interested in nurses with recent clinical experience- willing to consider strong experience from the following specialties for this role: Hospice, Homecare, ED or ICU Job Description: · Will be operating in a Case Manager capacity versus hands on work (plan of care, ordering medication, etc) · Nurses here will not be working in a hospice facility but will be tasked with visiting Hospice patients at various patients homes. · Will have to drive their own vehicle (will be reimbursed for mileage) · Will NOT need to drive patients Case Load: · 4-6 visits per day. Often, many patients located within same facility. Contract Length: 13 wks (+ability to extend) Number of Openings: 2 Start Date: 1/20 Hours/Shifts/Schedules: M-F 8a-5:00pm! No weekends! Other Details: + Zoom interviews! + EMR – EPIC (will train) + Case Management Experience NOT REQUIRED – WILL ACCEPT HOSPICE EXPERIENCE ONLY (Case management is a major bonus though). + Holiday coverage - one summer holiday The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.
