Case managers are responsible for determining the needs of the member and coordinating services to ensure that needs are met in an individualized manner.Case managers coordinate multiple services for members through comprehensive assessment, care plan development, care plan implementation, follow-up activities, and referrals to other needed services.Case Managers will work with the State and the Oregon/Clackamas County to provide Medicaid Assessment.Collaboration with internal and external stakeholders.Communication activities which include telephone interactions, caregiver interactions, rounds, interdisciplinary team meetings, and other related evidence-based practices.Referrals and related activities (such as scheduling appointments for the individual) to help the eligible individual obtain needed services.Ensuring quality driven outcomes through best practice and motivational interviewing.

Additional Skills & Qualifications

Must reside within the State of Oregon.Active unrestricted Oregon Registered Nurse (RN) License.Ability to be licensed for the State of Nebraska, if you don't currently hold a compact license.Bachelor's Degree from an accredited college or university in a related field.2+ years of Case Management experience.Knowledge of medical records, medical terminology, and disease processes.Strong clinical assessment and critical thinking skills.Excellent written and verbal communication skills.

Pay and Benefits

The pay range for this position is $36.00 - $43.00/hr.
