Avera is currently seeking a RN Case Manager – Care Transitions looking to deliver the best possible nursing care in Avera’s patient-focused settings.

The RN Case Manager – Care Transitions manages the care of patients referred to the Care Transitions program. Performs patient assessment and develops a patient-centered plan of care including follow-up phone calls and home visits. Addresses clinical concerns, provides education, makes referrals to appropriate resources and services, assists with access to care, improves care coordination and assures an effective discharge plan for patients at the end of the Care Transitions service. The RN Case Manager – Care Transitions will develop patient care plans in accordance with established protocols. Maintains contact with referring facilities, agencies, and community resources.Education and/or Experience
Graduate from a CCNE or NLN accredited nursing program is strongly preferred. Graduate from an approved nursing program is acceptable.Licensure/Certification/Registration
License as a Registered Nurse (RN) in South Dakota and/or meeting the requirements of Multi-state licensure is required.Work Schedule
Monday-Friday, 8:00am-5:00pm; 48 hours every 2 weeks Weekend rotation: every 4th Every 3rd holiday On-call: Minimal hours to be available for LPNsPay
The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$29.50 to $44.25
