RN Case Manager - Case Management - Full Time Days 7:30 am - 4:00 pm and rotating weekends Department: Case Management  Schedule: Full Time Days 7 am - 4:00 pm Monday - Friday and rotating weekends Hospital: Henry Ford Health Genesys  Location: Grand Blanc, Michigan GENERAL SUMMARY Creates plan for care across the continuum, integrating patient/family preferences and values. Monitors patient medical necessity and level of care through assessments, ongoing evaluations and/or patient records. Advocates for resources and removal of barriers. Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan. Acts as a resource for adequate medical record documentation, level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care. Education/Experience Requirements  Licensed Registered Nurse credentialed from the Michigan Board of Nursing required.  Professional Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association preferred

Preferences:

RN Case Manager/Navigator experience helpful Hospital experience helpful Acute Care Nursing Experience 2-3 years highly preferred. Multitasking, Good Communication and Computer Literacy highly preferred.
