Sentara Hospice is currently hiring a full time RN Case Manager to join their Newport News team.  

Shift: Monday - Friday, 8am - 5pm

Travel Within: Newport News, Hampton, Williamsburg, Gloucester, Mathews, New Kent

The Registered Nurse develops, implements and evaluates plans of care for specific patient populations. Coordinates and collaborates with multidisciplinary team members to facilitate integrated and comprehensive care. Possesses clinical knowledge and skills to meet standards as required by specific clinical areas.

Requirements:

RN-Associate's Degree ORRN-Bachelor's Level Degree ORRN-Diploma (Non-degree) ORRN-Doctorate Level Degree ORRN-Master's Level DegreeRequired: Basic Cardiac Life Support1 year Nursing experience preferred
