RN Case Mgr I - Ambulatory
Job Summary The RN Case Manager coordinates care for Newborn and High Risk Programs. This includes but is not limited to collaboration with families, internal/external medical teams, DHS/CUAs, DME companies, Home Care Companies, Ride share, etc. The RN Case manager performs chart reviews and transition of care. Detailed documentation in electronic health records. Excellent customer service, excellent communication skills both in person, and telephonically. Build and/or maintain rapport with patient/families, providers and team members alike. Ability to triage and route patients/families accordingly. Ability to multi-task. Adherence to HIPAA.
