RN - Certified Diabetes Care and Education Specialist (CDCES)
Full Time - Marrero, LA
The Coordinator of Diabetes Center assesses, plans, implements and evaluates the activities of the Diabetes Center based on evidence-based standards of care and the organizational policies.
Required: Certified by the Association of Diabetes Care and Education Specialists (ADCES)
General Duties:
1. Develop DSMT (Diabetes Self-Management Training) aka DSMES (Diabetes Self-Management Education and Support) accredited program to meet the education needs of patients at WJMC.
2. Assist providers and patients to develop treatment plans to manage diabetes.
3. Collaborate with providers on changes to treatment plans based on examination of blood glucose logs and continuous glucose monitor and insulin pump data.
4. Communicate treatment plan changes to patients and care teams.
5. Guide patients to community resources to support implementation of their treatment plans.
6. Maintain knowledge of and adhere to clinical practice guidelines established by The American Association of Clinical Endocrinology (AACE), American College of Endocrinologists (ACE), ADCES, and policies and procedures of WJMC.
7. Collaborate with hospital and clinic staff in the development and implementation of standards of practice in the care of patients with diabetes. Identify progress toward desired care outcomes and coordinate multi-disciplinary team interventions to move toward desired outcomes.
8. Evaluate effectiveness of patient care and education relative to diabetes. Institute program changes based on findings to promote continuous quality improvement.

2 years Diabetes management in inpatient and outpatient healthcare setting
