GENERAL OVERVIEW:
Registered Nurses (RNs) in this role work in a high-volume call center environment, serving as the primary point of contact for physicians and referring healthcare providers seeking assistance with patient transfers within the Allegheny Health Network (AHN). Key responsibilities include the efficient triage and facilitation of complex requests (telemedicine, consults, transfers, and emergency alerts), ensuring timely patient care transitions, and effective collaboration with diverse clinical teams to maintain patient safety and optimal care delivery .
ESSENTIAL RESPONSIBILITIES
Coordinate all aspects of patient care for adult and pediatric transfers, working collaboratively with the interdisciplinary Transfer Center team. Responsibilities include facilitating timely transitions, ensuring high-quality care, optimizing patient experience, and gathering comprehensive clinical data. This encompasses obtaining verbal reports from referring providers, accessing and reviewing electronic health records (EHR), and reviewing relevant clinical laboratory and diagnostic results. 25%
Facilitate paitent throughput within and into the AHN network by, coordinating communication between referring facility providers and the accepting AHN providers and clinical management team. Identifying the most appropriate AHN facility destination location meeting the patient’s clinical needs, and assuring that beds are promptly assigned based on patient clinical condition and hospital and departmental protocols.20%
Identify and prioritize the needs of incoming callers to provide patient-centered support, guiding them through the transfer process with education, screening, referrals, care coordination, and any additional assistance required 10%
Simultaneously utilize several software platforms and a multi-channel phone system to maintain accurate patient records, coordinate timely access to care, and manage seamless communication between various clinical departments and external facilities. 15%
Clinically prioritize and coordinate patient placement within the AHN network, ensuring optimal allocation of resources based on patient acuity, bed capacity, and staff availability10%
Provide clinical direction and support to non-clinical staff to enhance hospital efficiency, including but not limited to: prioritizing inbound patients, coordinating bed allocation, and facilitating timely patient placement.10%
Monitor department patient boards, prioritizing bed placement for critical or high-priority transfer patients. Receive and document updates on patients' clinical conditions within the electronic health record (EHR), maintaining continuous monitoring until the transfer request is finalized.10%

QUALIFICATIONS:
Required
Associates or Diploma in Nursing or relevant experience and/or education as determined by the company in lieu of bachelor's degree.
5 years of experience in Clinical Nursing
3 years of experience in stepdown/critical care/Emergency Roon
PA license
Preferred
Bachelor's degree in Nursing, strongly preferred
SKILLS:
Computer literacy - Ability to simultaneously work within, several software packages, multiple screens, and a multichannel phone system
Communication/Collaboration - maintain focus in fast-paced environment and support collaboration between providers, clinical staff, facility leaders, and non-clinical transfer center team members
Decision-making - demonstrated critical clinical decision making and leadership skills needed in a high-pressure healthcare environment
Composure - demonstrated ability to juggle multiple priorities in a fast-paced environment
