Job Summary Novant Health is seeking an RN Community Liaison - HomeCare The RN Community Liaison serves as a leader of the healthcare team at Novant Health by establishing and maintaining positive relationships with customers and referral sources, such as Physicians, Practice Groups, Facilities (SNFs, Rehabilitation Centers, Retirement Homes, Assisted Living Facilities, and Adult Family Homes). Responds to customer requests and concerns. Provides education to community and all potential referral sources on home health service offering, eligibility criteria, and referral process. Responsibilities: + Establish and maintain a good working relationship with physicians, community referrals sources hospital staff, discharge planners and business associates. + Provide excellent customer service at the point of referral and assures a smooth transition from hospital or SNF discharge to admission to home health. + Coordinate with referral sources to assure a smooth process for admission of patients either directly from the physician office, facility or from home. + Assist family or referral source in coordinating the order any identified equipment or supplies. + Assist in identifying any patients readmitted to hospital and coordinate with NHRMC Home Care facility based liaisons and discharge planners to assure smooth resumption of services upon discharge from the hospital. + Plan and attend marketing functions and community education events to enhance community awareness. + Educate physicians and their office staff and community of all services offered by NHRMC Home Care. + Assist in marketing plan development. + Maintain accurate records of patient and referral contacts. + Demonstrates Standards of Performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence Remarkable Care. Qualifications + Education: High School Diploma or equivalent required. Bachelor’s degree in a health related field preferred. + Licensure: Valid Driver’s License required.
