General Summary
Transplant Coordinators coordinate all aspects of care for both transplant recipients and donors. A coordinator participates in medical, administrative, research and teaching duties. The coordinator is an active member of the multi-disciplinary transplant team.
a Pre-Liver Coordinator, your primary responsibility will be to collaborate with the healthcare team to ensure comprehensive care for patients awaiting liver transplants. This includes coordinating pre-transplant evaluations, managing patient records, and communicating with patients and their families to provide support and education throughout the process. You will work closely with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans for each patient. Your role is crucial in ensuring that patients receive the best possible care and are well-prepared for their transplant. Your dedication and attention to detail will help improve patient outcomes and enhance the overall efficiency of our transplant program.
The Pre-Liver Coordinator position is a full-time role, with working hours from Monday to Friday, 8:00 AM to 4:30 PM. There are no holiday or weekend shifts required. After one year of working in this role, there is an on-call portion that can be done from home. Additionally, after six months, there is an option to work remotely one day a week managers discretion.
EDUCATION/EXPERIENCE REQUIRED
Bachelor's degree in nursing preferred. Organizational and Interpersonal skills to effectively communicate with patient care personnel and relate to the patients and their families. Minimum of two (2) years clinical nursing experience in an inpatient or outpatient setting. Analytical ability and motivation necessary to effectively plan, deliver and implement plans of care to in acute and outpatient situations. For those working as a Coordinator of Heart Transplant/with VADS: Experience in ICU or preferred.
CERTIFICATIONS/LICENSURES REQUIRED
Current RN license in the State of Michigan
