REGISTERED NURSE DIABETES CARE AND EDUCATION SPECIALIST - DIABETES CARE CONNECTION - 40 HOURS - DAY SHIFT - POPULANCE

This position will require in person work at various locations throughout the Jackson and surrounding areas, as well as some non-business hour availability to accommodate patients during evenings or weekends.
GENERAL SUMMARY:
The Diabetes Care Connection Service within Population Health Management provides an array of comprehensive interventions that are designed to optimize health outcomes for patients with diabetes. The Diabetes Care and Education Specialist (DCES) for Population Health is an integral and autonomous member of the patient-centered care team that participates in the development and implementation of innovations in care delivery by providing collaborative, comprehensive, person-centered care, and education that is conducive to behavior changes that lead to improved quality of life across an individual's lifespan. The DCES promotes self-management support to patients by enhancing their abilities to practice healthy behaviors that reduce risks often associated with poor disease control and progression. The DCES provides support to all members of the care team by serving as an expert resource knowledgeable in current medical treatment modalities for patient with diabetes.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, cardiometabolic conditions, and self-management support strategies of diabetes into clinical practice. Completes comprehensive physical, emotional, and social assessments for each patient. Using this information, develops an individualized care plan based on the patient’s assessed needs and goals. Documents all individual contacts/visits in the Electronic Health Record and outcomes database according to departmental guidelines in a timely manner. Applies the Association of Diabetes Care & Education Specialists 7(AADE7) self-care behaviors to educate and initiate behavior change in patients with diabetes. Uses this knowledge to develop/update curriculums according to the National Standards of Diabetes Self-Management and Support Guidelines.This posting represents the major duties, responsibilities, and authorities of this job, and is not intended to be a complete list of all tasks and duties. EDUCATION/EXPERIENCE REQUIRED:
Bachelor's degree in nursing required. Minimum (2) two years of clinical experience. Excellent verbal communication and written documentation skills. Excellent customer service and interpersonal skills including the ability to interact with internal and external customers and all levels of the organization. Strong problem-solving, analytical, and decision-making skills. Strong computer skills and knowledge. Understanding of social determinants of health and their impact on a patient's wellbeing. Strong organizational, planning and implementation skills with the ability to handle multiple complex patients' needs simultaneously. Strong sense of compassion with the ability to successfully advocate for patients and their families.CERTIFICATION/LICENSURES REQUIRED:
Registered Nurse with a valid, unrestricted license in the State of Michigan required. Certification as a Diabetes Care and Education Specialist by the Certification Board for Diabetes Care and Education preferred. For new hires, certification must be obtained within 18 months from date of hire.
