This unique position partners Behavioral Health and Care Coordination by conducting evaluations of patients in psychiatric crisis for disposition planning as well as case management services to those patients in need. The Evaluator RN also reviews transfer requests for admission to Hospital Programs. Responsibilities may include case and crisis evaluation, care planning, order management, and community resource coordination to patients and their families who utilize inpatient and outpatient medical services. Work involves close coordination between external organizations and hospital programs. Work involves the use of sophisticated interviewing, assessment, and multi-modality interventions aimed at positively affecting the physical, cognitive, and emotional functioning of patients and their families.
