In this inspiring role, you hold the power to perform comprehensive assessments, collaborating seamlessly with our multi-disciplinary team of experts, including PT, OT, ST, MSW, and paraprofessionals. Together, we will work passionately to achieve the heartfelt outcomes and goals of our patients, leading them on a journey of healing and hope.
Supervise your caseload, nurturing the growth and development of LPNs and Home Health Aides, as we cultivate a team of caregivers. In your role as a facilitator of care coordination, we celebrate the unity that arises when PRN staff steps in, ensuring seamless support for your patients. We welcome you to a role you can grow in!
Qualifications
Active RN License - Embark on a Thriving Career as a Licensed RN!One (1+) Year of Clinical Excellence CPR CertificationValid Driver's License
