RN Case Manager - PACE/ElderPlace

Schedule: Part Time, 24 hours/week; Saturday and Sunday 8am-8pm

Job Description:

The Community Care RN provides and supervises professional, independent nursing care for Providence patients. Care is based in the nursing process and is consistent with the Oregon Nurse Practice Act. Applies knowledge of assessing, planning, ordering, giving, delegating, teaching and supervising professional nursing care delivery that promotes patients' optimal health and independence. All Providence professional nursing practice requires a broad range of general nursing knowledge and expertise.

Required Qualifications:

Relevant training applicable to the position, as specified by the Nurse Manager. Upon hire: Oregon Registered Nurse License Within 30 days of hire: National Provider BLS - American Heart Association Upon hire: Relevant certifications applicable to the position, as specified by the Nurse Manager

Preferred Qualifications:

Bachelor's Degree Nursing. 1-year home health RN experience.
