Skilled VisitsDay-to-day patient interactionEducation on Health Conditions, such as CHF, CODP, DiabetesWound CareMedication ManagementChanges dressings, bandages and contraptions as prescribedDocumentation of patient’s condition and pointing out significant changes, to be reported to the physician.Working alongside other nurses, facility staff, physicians, intake coordinators, and hospice consultantsRequirements:
