RN - Homecare Nurse - Grafton MA

Homecare Nursing Position with a young adult patient in Grafton MA. 1to1 flexible nursing care on Monday-Sunday,1p-11p and 11p-7a. shifts available, full time and part time with INCREASED RATES. G-tube and central line experience highly recommended!


Essential Duties and Responsibilities:

Assesses and evaluates patient nursing needs regularlyUtilizes the nursing process to plan, implement, and evaluate patient/client care and/or services provided to client(s) receiving contracted services, as requiredInitiates the plan of care when directed and requests from prescriber any necessary revisions as patient status and needs changeIncorporates therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goalsIntervenes as applicable in response to changes in patient/client condition and reports such changes to appropriate physician or supervisorAdministers medications and treatments as prescribed, as well as, monitors and documents effectiveness of interventionAssures patient/client safety at all timesMonitors the safety and effectiveness of the care/services environment and equipment used, including universal precautionsReviews plan of care and program protocols and identifies areas for healthcare teaching/education, care coordination, goal setting and discharge planning needs, as applicableDocuments all physician collaboration/coordination of care in the medical record and communicates this information as neededAdheres to the following professional practice standards:Nurse Practice ActStandards of Nursing PracticeStandards of Patient CareStandards of Accrediting BodyState and Local LawsPatient Bill of RightsPolicies and procedures of contracted customersAdheres to Maxim policy, procedure and applicable protocolsAttends case conferences, team meetings and education in-services, as requiredPerforms other duties as assigned/necessary

Minimum Requirements:

Graduate of an accredited school of nursing preferredCurrently licensed as a Registered Nurse in specific state requiredComplies with accepted professional standards and practiceOne year prior professional nursing experience preferredCurrent CPR, as applicable, for direct patient carePPD or chest x-ray if applicableSuccessful completion of Maxim Orientation and Compliance training, and all other assigned trainings as applicable requiredSuccessful completion of Maxim’s Competency ProgramProficiency in the English language requiredCurrent Health Certificate (per facility contract or state regulation) requiredMust meet all federal, state, local and contract requirements
