This position will be working 32 hours/week on day time/variable shift and rotating weekend/holiday coverage.

The Hospice Access Field and Triage RN will complete medical requirements while coordinating a whole-person plan of care. Care is according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act.

Required Qualifications:

Education to meet certification, license or registration requirement. Upon hire: Oregon Registered Nurse License  Washington Registered Nurse License - to be obtained within 2 months. Two years: Medical/surgical experience in an acute care setting, and/or a combination of medical/surgical, long term care or home health or hospice experience. Experience using electronic medical record software.
