Overview:

We are seeking an experienced and compassionate Registered Nurse (RN) to join our pediatric Hematology Oncology team as an Inpatient Liaison. The RN Inpatient Liaison will play a pivotal role in ensuring high-quality, coordinated care for our pediatric patients and their families as they navigate their healthcare journey. This position requires strong clinical skills, exceptional communication abilities, and a passion for working with pediatric populations.

Key Responsibilities:

Clinical Support: Serve as the primary nursing liaison between inpatient units, outpatient clinics, and the Hematology Oncology team. Assess, monitor, and advocate for pediatric patients’ medical needs during their inpatient stay.

Care Coordination: Collaborate with multidisciplinary teams to develop and implement individualized care plans for patients. Facilitate smooth transitions between inpatient and outpatient care, ensuring continuity and quality of care.

Patient & Family Education: Provide comprehensive education and support for patients and their families regarding treatment plans, medication management, procedures, and self-care strategies. Address any questions or concerns to promote understanding and adherence.

Communication: Maintain open lines of communication with families, medical staff, and other healthcare professionals. Participate in rounds, case discussions, and team meetings to ensure cohesive care delivery.

Crisis Intervention: Identify and respond to any patient crises or changes in condition promptly. Utilize clinical judgment to escalate care as necessary and ensure appropriate interventions are implemented.

Quality Improvement: Contribute to quality improvement initiatives within the department by participating in data collection, analysis, and reporting. Engage in program development that enhances patient care and staff education.

Documentation: Maintain accurate and thorough documentation of patient assessments, care plans, communications, and interventions in accordance with facility policies and regulations.

Advocacy: Act as an advocate for patients and families within the healthcare system, working to address concerns and facilitating access to additional resources.

Qualifications:

Requires an Associates of Nursing (ASN). Bachelors of Nursing (BSN) preferred. Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license. Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred. Requires Basic Life Support certification through the AHA. Other advanced life support certifications may be required per unit/department specialty according to patient care policies. Specialty certifications may be required per unit/department specialty in accordance with the department provision of care. Requires knowledge of personal computers using various types of word processing and database software. Requires clinical work experience, medical terminology. Requires understanding of medical record requirements, regulations and policies.
