PURPOSE OF POSITION
The primary purpose of the Skin Team Nurse is to provide clinical support and coordination of patient care, provide patient and family teaching and work cooperatively with the multidisciplinary care team in maintaining standards of care within the framework of the Ohio Nurse Practice Act. The Skin Team Nurse provides current evidence-based practice in relation to skin, wound and ostomy needs of patients at Aultman Hospital.
RESPONSIBILITIES & EXPECTATIONS
Coordinate the skin and ostomy care of patients under the direct supervision of the Unit Director of Skin team in collaboration with the multidisciplinary care team. Assist in Aultman’s quarterly skin prevalence audit, collect data and report findings. Provide organizational education, develop/revise policy and procedure of current skin related evidence-based practice. Participates in staff and management meetings as they relate to QI/PI activities.Obtains pertinent patient information including vital signs, assessment/measuring of various wound types, patient’s medical/surgical history, and changes to medication regimen. Provides appropriate documentation in the patient’s chart/EMR (electronic medical record). E.g. Treatment charting, patient history/medication history, assessment and recommendations of care. Ability to provide application of various wound care products/dressings/equipment. Notifies physician immediately of abnormal test results, and complications that may arise with a patient’s wound. Reinforces/facilitates patient education appropriate to the individual patient’s needs. Ability to assist physician with preparations for various procedures, but not limited to e.g. Skin substitute application, debridement, wound cultures, and cleansing of the wound bed prior to treatment. Ability to apply ordered topical medications prior to treatment. Follows universal precautions and adhere to infection control policy/procedures. Serve as a member of Aultman’s health care team that provides quality care to patients and maintain good working relationship with physicians and other members of the health care team. Performs other duties as assigned by the Skin Team Unit Director Participates in continuing education and professional growth. Attends staff meeting Maintains knowledge of, and complies with, nursing, medical, and clerical policies and procedures. Adheres to the standards and policies of Aultman Health Foundation. Job Requirements
Graduate of an accredited school of nursing – Education level must be at least at the level of education required for a Registered Nurse (RN) Current licensure to practice as a nurse in the State of Ohio; BLS certification Must obtain and maintain WCC (Wound Care Certified) certification within 18 months of hire. Must obtain and maintain Ostomy Certification within 18 months of hire. Demonstrates proficiency in nursing knowledge and skills Demonstrate commitment to the delivery of ethical nursing practice, as defined by the ANA Code of Ethics Demonstrates ability to deliver evidence-based nursing care according to national and specialty practice guidelines and current nursing research Demonstrates ability to practice and collaborate with other disciplinesWORKING CONDITIONS:
Various start times: 8-hour shifts, Mon.-Friday. or hours as required by the job. Potential weekend call rotation. Works collaboratively with Wound Care physicians, referring physicians and other disciplines. Subject to frequent interruptions and changes in priority of duties throughout the day. Sitting/standing/moving/ lifting about during working hours
