We are now hiring Part-Time & PRN LPN's & RN's for various nursing positions:

School Nurse Facilities Hospice Visits

Schedule:

Weekend shift Monday-Friday

Your Job Duties:

Review weekly care notes for quality of documentation, changes in patient condition requiring revisions to the Plan of Care. Case management: contact families, coordinate care, communicate changes in Plan of Care. Obtain physician's signature approval as necessary. Administer prescribed medications and treatments in accordance with approved nursing techniques Observe patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments and significant incidents Take and record patients' vital signs to detect deviations from normal and assess condition of patient Respond to life saving situations based on nursing standards, policies, procedures and protocol Document nursing history and physical assessment for assigned patients Initiate a patient education plan according to the individualized needs of the patient as prescribed by the physician and/or Agency policy including patient and family instruction Participate in the implementation and evaluation of patient care
