What to expect as a Case Manager
As a Case Manager, you'll lead expert interventions and collaborative efforts to ensure quality outcomes for managed care patients. Conducting in-person assessments on admission, you'll seamlessly document critical information in the medical record. Your negotiation skills will be crucial in facilitating timely access to necessary care and securing appropriate reimbursement from payers. Additionally, you'll play a key role in developing, documenting, and coordinating alternative discharge plans from the outset, contributing to a comprehensive and streamlined continuum of care.
Minimum Qualifications:
Nursing Degree from an accredited college or university; BSN preferred.
Current, unencumbered, active RN license in the state.
Three years of clinical experience in an acute care setting or long-term care; one year of post-acute care or case manager experience preferred.
Previous MDS and admissions experience and working with interdisciplinary team preferred.
Supervisory experience is a plus.
