Responsibilities
The Nurse Navigator provides clinical and emotional support to cancer patients. The Navigator partners with patients, families, interdisciplinary teams and community resources to provide well-coordinated, timely, compassionate and exemplary interdisciplinary care. This involves, in part: *Working closely with medical specialists in accumulating results and conveying difficult outcomes to patients. *Being readily available for consultation with patients throughout the treatment process, answering their questions and
providing emotional support during exams and procedures when requested. *Functioning as patient liaison and advocate. *Facilitating appropriate follow-up care for positively diagnosed individuals. *Developing close working relationships with the hospital teams, cancer specialists and their support staff, referring providers and cancer support services. *Participating in weekly multidisciplinary case conferences.

Qualifications
Minimum:
• 3 - 5 years of full time experience as a RN with critical thinking and working with high-acuity patient populations
Licensure:
Current and active CA RN licenseCurrent BLS certificationCurrent Chemotherapy / Biotherapy provider card within 90 days of hireOncology Certified Nursing (OCN) certification, within two years of hireEducation:
BSN or equivalent combination of education and experience
